Welcome to Chappaqua Central School District Athletic Registration for Horace Greeley High School, Seven Bridges Middle School, and Robert E. Bell Middle School. To be cleared to play, you must complete this form and upload a copy of an updated physical form. Physicals are only valid for 365 days from the date of the physical exam.
ALL MEDICAL INFORMATION REQUIRED FOR REGISTRATION MUST BE UPLOADED ON THIS SITE.
New users will receive a confirmation email from "Chappaqua CSD Sports Registration" with a link to verify your account. Once verified, you will be able to complete your child's registration. Once the registration is complete, you can view your account to check for account status- "PENDING" means that your registration is active and waiting for the nurse's review and approval. Once your registration has been reviewed by the nurse, you will receive a separate email stating your child has been APPROVED to participate or NOT APPROVED (along with the information that is needed for approval). Please note that the nurses typically do not start reviewing forms until after the registration portal has closed.
YOUR ATHLETE IS NOT CLEARED TO PARTICIPATE IN TRY-OUTS, GAMES, OR PRACTICES UNTIL YOU RECEIVE THE EMAIL THAT SAYS THEY HAVE BEEN APPROVED. Please note that submitting this registration does not guarantee approval. Your approval status can be checked by viewing your account and final approval will be communicated by email from Chappaqua CSD Sports Registration.
